You must accept these Terms and Conditions of Use to access “Main Line Health MyChart” online.
Main Line Health MyChart is an internet application that allows a patient to have web-based access to portions of their Main Line Health (MLH) electronic medical record. Main Line Health MyChart also allows a convenient communication channel with MLH for non-urgent messages.
Use of the Main Line Health MyChart service is optional.
Your login username and password are important to keeping your information confidential and protecting your personal information. It is your responsibility to keep your username and password confidential. Your username and password are only known by you and can be changed online at any time by you. Your password is not known by anyone at MLH.
Main Line Health MyChart Terms and Conditions of Use: Your Responsibilities.
Please review the following Terms and Conditions of Use. You must observe the Terms and Conditions of Use described below when using Main Line Health MyChart.
I AGREE THAT:
1.I must be age eighteen (18) or older to request a personal account to use Main Line Health MyChart.
2.I will not share my username and password with anyone.
3.Main Line Health MyChart is a communication service offered as a convenience to me. I understand that it provides me online access to portions of my MLH medical record and that I am solely responsible for any sharing of its content that I intentionally or unintentionally communicate to others.
4.I may receive messages, test results and other communications delivered via My Main Line Health MyChart. I understand that I may receive test results before my MLH physician/health care provider contacts me or reviews the test results. If I have questions or concerns regarding test results that were made available to me via Main Line Health MyChart, I should contact my physician/health care provider’s office.
5.I will be notified by e-mail when there is new medical information to be viewed on Main Line Health MyChart. This means that any person with access to my e-mail account will be able to see this notification. This could include my spouse, employer or anyone else that can access my email account. Although no private medical information will be included in the e-mail, the notification that new medical information is available by accessing Main Line Health MyChart may be information that I would not want others to know. Thus, I should take this into consideration when providing an e-mail address.
6.If I choose to authorize any individuals to have “proxy” access to my Main Line Health MyChart portal, those individuals will have access to all of my health information that is available in my portal. This includes but is not limited to mental health treatment information, substance use disorder treatment information, genetic information and reproductive health information. I will periodically review who is authorized to have “proxy” access to my portal and make changes if I no longer wish for them to have access.
7.I understand that if I choose to link or connect any third-party application to my Main Line Health MyChart portal account, any and all of my health information, including but not limited to mental health treatment, substance use disorder treatment, genetic and/or reproductive health information will be released to the third-party application.
8.Messages I send through Main Line Health MyChart may be routed to the appropriate department as necessary for handling and therefore MLH health professionals, administrators, and support staff who work with my physician(s)/health care provider(s) and their practice(s) may view my messages and be involved in addressing my message. Therefore, use of Main Line Health MyChart may not be appropriate for very sensitive information I wish to discuss directly with my physician/health care provider.
9.I will not send any messages through Main Line Health MyChart that require urgent or emergent attention. I will call 9-1-1 in the case of an emergency. I understand that turn-around time for responses is usually two (2) business days, but may vary by individual physician/health care provider or practice. MLH does not guarantee that I will receive a response to any particular message. If I have not received a response within a reasonable time and feel the message must be addressed, I should call my physician/health care provider or their office.
10.All communications through Main Line Health MyChart will be in regard to my own medical condition(s) and I may only ask for advice about me through Main Line Health MyChart. I understand that asking for advice on behalf of another person could potentially be harmful and is a violation of these Terms and Conditions for Use. MLH DOES NOT ASSUME ANY RESPONSIBILITY FOR MEDICAL INFORMATION OR SERVICES USED BY A PERSON OTHER THAN ME.
11.The contents of any messages will be stored in my permanent MLH medical record.
12.Neither Main Line Health MyChart nor any other application available through this site is a substitute for appropriate and timely contact with my physician/health care provider(s). This site is not a substitute for an office visit or consultation with my physician/health care provider(s).
13.Not all of my MLH physicians/health care providers use Main Line Health MyChart, so they may not be aware of the information that is in my Main Line Health MyChart. I understand that I may not be able to communicate electronically or view information from all of my MLH physicians/health care providers.
14.There may be some information that my MLH physicians/health care provider(s) believe should not be placed in my Main Line Health MyChart record. I may contact the MLH Health Information Management Department to request a complete copy of my medical records or if I believe there are any discrepancies with the information listed in my records.
15.I will not use inappropriate, profane, or threatening language in my messages sent from my Main Line Health MyChart.
16.I can un-enroll from my use of Main Line Health MyChart at any time by completing the appropriate online request to un-enroll or by calling MLH Portal Support. MLH may, in its sole discretion, discontinue my use of Main Line Health MyChart if MLH determines that I have violated these Terms and Conditions of Use or for any other reason or no reason. I will be notified if MLH discontinues my access to Main Line Health MyChart.
17.MLH may offer links to medical websites not managed by MLH. The website link(s) are for patient informational purposes only. MLH does not endorse and has not verified the accuracy of the information in/on these websites, and I should not rely on any information found on the websites for purposes of treatment or diagnosis.
18.By accessing or using Main Line Health MyChart, I am bound by all of these Terms and Conditions of Use. I know that MLH may modify these Terms and Conditions of Use at any time and I agree that such modifications are effective immediately upon posting of the modified version.
19.These Terms and Conditions of Use shall be governed by and construed in accordance with the laws of the Commonwealth of Pennsylvania. Additionally, the pages on this site are subject to MLH’s website general terms of use and web privacy statement, which can be viewed at www.mainlinehealth.org.
