This web site and related services are provided by Ascension Wisconsin to its patients subject to compliance with the terms and conditions set forth below. Keep in mind the use of MyChart is voluntary and you are not required to use MyChart or to authorize other persons (proxy) to access MyChart. Please read the following information carefully. Your continued use of this web site will indicate your agreement to be bound by the terms and conditions set forth below. If you do not agree to be bound by these terms and conditions, promptly exit this web site.
By agreeing to the following terms and conditions, I acknowledge that I am requesting access to portions of my personal health information and the ability to communicate with my Ascension Wisconsin (hereinafter referred to as Ascension) team concerning my health information via the Internet using an electronic application called MyChart. I hereby expressly authorize Ascension to disclose my identifying health information to my designated MyChart account for the purposes described herein. In providing this authorization, I acknowledge that I am requesting access to portion of my personal health information and the ability to communicate with my Ascension Wisconsin providers regarding my personal health information via the Internet using MyChart. My continued use of this site indicates my agreement to abide by these terms and conditions.
I understand that MyChart should never be used for urgent matters. Because the anticipated turnaround time for response to electronic messages may be several business days during posted business hours, I will contact my Ascension physician by phone or seek walk-in services if I have a matter that requires medical attention and cannot wait for a regular appointment. If I am experiencing a life-threatening illness or injury, I will go to the nearest hospital emergency department and/or dial 9-1-1 for immediate attention. Furthermore, with respect to any electronic communications, I understand Ascension will only be able to respond to such communications based on the information I provide to Ascension. If there is insufficient information provided, Ascension will be unable to ensure accurate and reliable services.
I understand that my Ascension health care team may send messages to me via MyChart. These messages may contain information important to my health and medical care, and it is my responsibility to continuously monitor these messages. By entering my valid and functioning email address, I have enabled Ascension to notify me of messages sent to my MyChart inbox. I acknowledge my responsibility to make sure a spam block doesn’t interfere with the messages and the messages are not blocked by my internet service provider. I will keep my email address updated. Ascension, its employees, and health professionals under contract to Ascension shall not be liable for any loss, injury, or claims of any kind resulting from MyChart messages I fail to read in a timely manner. I understand that I may or may not continue to receive letters from my physician(s) regarding my test results. If I have not received information, I may contact my physician directly for information. I may continue to contact my physician for any information or assistance that I may need.
I understand that all health-related information provided by this website, other than my personal health information, is intended to be for general use and should not be used as a substitute for a personal visit to a health care professional. MyChart contains limited medical information from my medical record and MyChart does not include the complete contents of my medical record. I understand that I can always request a paper copy of my medical record from Ascension and/or its affiliated clinics and that I may be charged a fee for such copies.
Because the information I obtain from the site may only be appropriate for general use but inappropriate for my own situation and could be misinterpreted, Ascension shall not be responsible for how I use the information obtained and shall not be liable for any damages or injury arising from that content. Any links to other sites are provided as merely a convenience to the users or this web site. You should consult with your physician or other health care providers regarding your own condition and how this web site content may or may not apply to you.
I agree that all communication through MyChart will be in regard to my own health condition and that the contents of any message may be stored in my permanent medical record. I understand that asking for advice or service’s on behalf of another person could be potentially harmful and is a violation of the MyChart terms of use. Ascension does not assume responsibility for and disclaims any and all liability in connection with health information or services on this website for anyone other than approved MyChart enrollees.
I understand that if I decide I no longer want to use MyChart ,I must call my primary care physicians office and request that my account be deactivated. I also understand that should I choose to inactivate access to my MyChart account this will in no way impact my care. I understand that should I inactivate access to my MyChart account, the inactivation will not apply to any information that has already been released via my MyChart account. I understand that when the Ascension and/or its affiliated clinics become aware of my death, access to my MyChart account will be canceled.
I understand that my use of MyChart is a privilege and not a right. I agree that if Ascension discovers that I have misused or abused MyChart access privileges in any way, including but not limited to misuse of the prescription request function, inappropriate requests/messages, threats to staff, and misuse of the proxy access, Ascension, at its sole discretion, may discontinue my participation in MyChart without prior notice and at any time.
As a patient, you have the right to access your medical records. We are providing access in our convenient online portal. Please know that some of your medical information may be available on the portal before it is reviewed by your clinician. Medical records should not be used as a replacement for in-person medical services. You should review them with your clinician as appropriate. Please note that certain types of documents will not be available for viewing on the online portal. You can get a full copy of your medical record. Or you can get help with questions about your information. Please contact the location where you received care and treatment.
